- summary of recovery history

- past and present addictions

- financial information, including, but not limited to, current income, assets, access to alternate funds, credit card or other payment processing information

Sensitive Information. When necessary, with your consent or as otherwise permitted by applicable law, we may process the following categories of sensitive information:

- credit history

- social security numbers or other government identifiers

- biometric data for purposes of background screening

- information revealing race or ethnic origin

In order to enhance our ability to provide relevant marketing, offers, and services to you and update our records, we may obtain information about you from other sources, such as public databases, joint marketing partners, affiliate programs, data providers, and from other third parties. This information includes mailing addresses, job titles, email addresses, phone numbers, intent data (or user behavior data), Internet Protocol (IP) addresses, social media profiles, social media URLs, and custom profiles, for purposes of targeted advertising and event promotion.

4. WHEN AND WITH WHOM DO WE SHARE YOUR PERSONAL INFORMATION?In Short: We may share information in specific situations described in this section and/or with the following categories of third parties. Vendors, Consultants, and Other Third-Party Service Providers.

We may share your data with third-party vendors, service providers, contractors, or agents ("third parties") who perform services for us, on our behalf, or on behalf of applicants or beneficiaries of Inservice Foundation’s financial assistance and require access to such information to do that work, subject to Inservice Foundation’s privacy and confidentiality practices. The categories of third parties we may share personal information with are as follows:

- Finance and Accounting Software and Tools

- Data Storage Service Providers

- Addiction recovery consultants and other support service providers, including, without limitation, counseling services


We also may need to share your personal information in the following situations:

- Business Transfers.

We may share or transfer your information in connection with, or during negotiations of, any merger, sale of company assets, financing, or acquisition of all or a portion of our business to another company.


5. HOW LONG DO WE KEEP YOUR INFORMATION?

We will keep your personal information for as long as it is necessary for the purposes set out in this privacy notice, or as long as needed for legitimate business purposes in line with industry practices, or as required or permitted by law (such as tax, accounting, or other legal requirements).You may request that your data be deleted at any time, but that may result in the termination of financial assistance by Inservice Foundation if you are receiving financial assistance. We have implemented appropriate and reasonable technical and organizational security measures designed to protect the security of any personal information we process. Inservice Foundation is committed to protecting the privacy of children and does not accept applications for persons under 18 years old (“minors”). Inservice shall not be liable for any unauthorized use of the Website by minors. Please contact us if you believe a minor has provided Personal Data to this Website without parental consent.

Withdrawing your consent: If we are relying on your consent to process your personal information, which may be express and/or implied consent depending on the applicable law, you have the right to withdraw your consent at any time. However, please note that this will not affect the lawfulness of the processing before its withdrawal nor, when applicable law allows, will it affect the processing of your personal information conducted in reliance on lawful processing grounds other than consent.


Opting out of marketing and promotional communications: You can unsubscribe from our marketing and promotional communications at any time by clicking on the unsubscribe link in the emails that we send, replying "STOP" or "UNSUBSCRIBE" to the SMS messages that we send, or by contacting us using the details provided in the section "HOW CAN YOU CONTACT US ABOUT THIS NOTICE?" below. You will then be removed from the marketing lists. 9. CONTROLS FOR DO-NOT-TRACK FEATURES


Most web browsers and some mobile operating systems and mobile applications include a Do-Not-Track ("DNT") feature or setting you can activate to signal your privacy preference not to have data about your online browsing activities monitored and collected. At this stage no uniform technology standard for recognizing and implementing DNT signals has been finalized. As such, we do not currently respond to DNT browser signals or any other mechanism that automatically communicates your choice not to be tracked online. If a standard for online tracking is adopted that we must follow in the future, we will inform you about that practice in a revised version of this privacy notice.


10. DO CALIFORNIA RESIDENTS HAVE SPECIFIC PRIVACY RIGHTS?


In Short: Yes, if you are a resident of California, you are granted specific rights regarding access to your personal information.


California Civil Code Section 1798.83, also known as the "Shine The Light" law, permits our users who are California residents to request and obtain from us, once a year and free of charge, information about categories of personal information (if any) we disclosed to third parties for direct marketing purposes and the names and addresses of all third parties with which we shared personal information in the immediately preceding calendar year. CCPA Privacy Notice


The California Code of Regulations defines a "resident" as:

Receiving help through Seeking or receiving financial assistance, facilitation of resources and supports, or other assistance from Inservice Foundation, its staff or volunteers;

- Participation in customer surveys and Facilitation in the delivery of our Services and to respond to your inquiries.

Will your information be shared with anyone else?


We may disclose your personal information with providers or other third parties who may provide services, supports or resources to beneficiaries of Inservice Foundation’s financial support (“Providers”)pursuant to a written contract between us and each service provider.

Providers are individuals and entities that may require personal data in order to assist beneficiaries of Inservice Foundation’s financial support. Service providers will be required to follow the same strict privacy protection obligations mandated by the CCPA.


We may use your personal information for our own business purposes, such as for undertaking internal research for technological development and demonstration. This is not considered to be "selling" of your personal information.


NPA Foundation has not sold or shared any personal information to third parties for a business or commercial purpose in the preceding twelve (12) months.


Your rights with respect to your personal data.


Right to request deletion of the data — Request to delete.


You can ask for the deletion of your personal information. If you ask us to delete your personal information, we will respect your request and delete your personal information, subject to certain exceptions provided by law, such as (but not limited to) the exercise by another consumer of his or her right to free speech, our compliance requirements resulting from a legal obligation, or any processing that may be required to protect against illegal activities.


Right to be informed — Request to know.


Depending on the circumstances, you have a right to know:

- whether we collect and use your personal information;

- the categories of personal information that we collect;

- the purposes for which the collected personal information is used;

- whether we sell or share personal information to third parties;

- the categories of personal information that we sold, shared, or disclosed for a business purpose;

In accordance with applicable law, we are not obligated to provide or delete consumer information that is de-identified in response to a consumer request or to re-identify individual data to verify a consumer request.


Right to Non-Discrimination for the Exercise of a Consumer’s Privacy Rights.

We will not discriminate against you if you exercise your privacy rights. However, Inservice Foundation may be unable to provide ongoing financial and other support or services if you are unwilling to disclose certain personal information.


Right to Limit Use and Disclosure of Sensitive Personal Information.


You have the right to withhold sensitive personal information, or to request that we delete or do not share personal information that you have shared with us. However, in certain situations, we may not be able to provide financial support for substance abuse recovery services without sensitive personal information.


Verification process.


Upon receiving your request, we will need to verify your identity to determine you are the same person about whom we have the information in our system. These verification efforts require us to ask you to provide information so that we can match it with information you have previously provided us. For instance, depending on the type of request you submit, we may ask you to provide certain information so that we can match the information you provide with the information we already have on file, or we may contact you through a communication method (e.g., phone or email) that you have previously provided to us. We may also use other verification methods as the circumstances dictate.


We will only use personal information provided in your request to verify your identity or authority to make the request. To the extent possible, we will avoid requesting additional information from you for the purposes of verification. However, if we cannot verify your identity from the information already maintained by us, we may request that you provide additional information for the purposes of verifying your identity and for security or fraud-prevention purposes. We will delete such additionally provided information as soon as we finish verifying you.


Other privacy rights,

- You may object to the processing of your personal information.

- You may request correction of your personal data if it is incorrect or no longer relevant, or ask to restrict the processing of the information.

- You can designate anauthorizedagent to make a request under the CCPA on your behalf. We may deny a request from anauthorizedagent that does not submit proof that they have been validlyauthorizedto act on your behalf in accordance with the CCPA.

- You may request to opt out from future selling or sharing of your personal information to third parties. Upon receiving an opt-out request, we will act upon the request as soon as feasibly possible, but no later than fifteen (15) days from the date of the request submission.
